Exclusive Partnership: Medically Home is a Virtual Hospital experience offered by the Cleveland Clinic.
- Our agency has partnered with Cleveland Clinic to provide top-quality RN, Therapy, and HHA care.
- Medically Home patients are seen two or three times per day by an RN to conduct a head-to-toe assessment, administer medications, and provide any necessary care.
This unique program lets our nurses deliver advanced, hospital-level care to patients in the comfort of their homes-an exciting opportunity to grow professionally while making a difference.

What You will Do
- Registered Nurse will provide individualized care and monitor patient condition with minimal supervision
- Registered Nurse will execute physician orders: administer meds, treatments, follow-up
- Registered Nurse will document patient care accurately and timely
- Registered Nurse will communicate patient needs and updates to physicians and care teams
- Registered Nurse will deliver emergency care as needed
- Registered Nurse will educate patients and families on prevention and wellness
- Registered Nurse will deliver advanced care to Medically Home patients in partnership with Cleveland Clinic, conducting multiple daily assessments and supporting a true hospital-at-home model
Schedule
- Variety of shifts available: Day, Evening, Night, Weekend, Holiday
- Position types: Part-Time, PRN/Per Diem
Requirements
- Current, unrestricted RN license in FL (or Compact)
- CPR certification
- Preferred: 1-year experience in home health
- Reliable transportation with active driver's license & auto insurance
- Ability to lift up to 50lbs and perform essential physical tasks
- Excellent critical thinking and assessment skills
- Preferred: BSN (Bachelor's in Nursing)
- Bi-Lingual a plus (not required)
